ACL / CCL tear recovery at home

What the first twelve weeks after a cruciate injury actually require, with or without surgery.

The short version

  • The canine 'ACL' is the cranial cruciate ligament, and tears are usually degenerative rather than traumatic.
  • Confinement is the hardest and most important part of recovery.
  • The opposite leg fails within two years in a large share of dogs, conditioning both sides matters.
  • Rehab beginning in the first two weeks produces better limb use at twelve weeks.

Set the house up before you need it

Recovery happens in a confined space: a pen or small room with non-slip flooring, no furniture to jump on, and no stairs. Set this up before the dog comes home, not after the first jump.

Get a sling or a support harness. A dog who cannot balance on three legs to toilet will otherwise be lifted badly, and owner back injuries during recovery are common.

Weeks one and two

The goals are pain control, incision protection, and preventing the dog from doing anything explosive. Leash walks are short and only for toileting.

Cold packing the joint for the first few days reduces swelling. Your surgeon or rehab practitioner will set the timing.

Passive range-of-motion work and gentle weight-shifting typically start now, guided by a rehab practitioner rather than improvised.

Weeks three to six

Controlled leash walking builds up in small increments, often adding a couple of minutes every few days. Slow walking on a leash loads the limb more evenly than trotting.

Underwater treadmill work commonly starts once the incision is fully healed. Sit-to-stand repetitions, cavaletti poles, and weight-shifting exercises come in as strength returns.

Weeks seven to twelve

Duration and difficulty increase; hills, figure-eights, and balance equipment appear. Off-leash running is still off the table.

Objective measures matter here, thigh girth compared side to side, and how the dog sits. A dog who sits with the operated leg kicked out to the side is still not loading it properly.

Protecting the other leg

A large share of dogs tear the second cruciate within a couple of years. Weight control is the strongest modifiable factor, followed by year-round conditioning rather than weekend bursts of activity.

Common questions

Can a CCL tear heal without surgery?
Partial tears in small dogs are sometimes managed conservatively with strict rest, weight loss, pain control, and rehab. Full tears in medium and large dogs usually do better surgically.
When can my dog use stairs again?
Typically not before eight weeks, and then only slowly and on leash. Your surgeon sets the timeline.
Is a brace an alternative to surgery?
Braces are sometimes used when surgery is not an option. Fit and compliance are the limiting factors, and evidence is weaker than for surgery.
